You will usually receive your support plan and referral codes at the end of your assessment. You can then contact providers and start organising services.

* Some providers will let you manage your own home care services under SAH. However, if you choose to do this, there is more work involved for you. We recommend that you carefully consider whether a self-managed home care package is the right option for you.
